MCR Health is committed to providing quality care to all our patients in a safe and professional environment. Our goal is to address each patient’s concerns on an individual basis and in a timely manner. Patient feedback is important to us.
If you are a patient of MCR Health and would like to file a complaint, you may do so by using the form below. You may also contact the corporate office and speak with the Patient Relations Coordinator by calling 941-776-4000 ext. 242. While we will make every effort to resolve patient complaints, any unresolved complaints will be forwarded to MCR Health’s Senior Management for review and action. All patient complaints will be returned within two business days.
