New Patient Process

Thank you for considering MCR Health as your primary care medical home. MCR Health is committed to providing quality care to all of our patients and we are excited for the opportunity to serve you.

MCR understands that choosing a new provider can be stressful. Just know, we will be with you every step of the way. Here’s what you need to do:

1. Choose a Provider

Browse our list of providers and locations to determine the best fit for you.

2. Schedule Your Appointment

Now that you have selected your provider, call their respective office to schedule your new patient appointment or you can fill out the Request an Appointment form. In the rare case a provider’s schedule doesn’t work for you, our patient services team will assist you in finding the right provider and confirming an appointment time for you.

3. Complete Your Pre-Registration

Two days prior to the scheduled appointment, you will receive an email to “Pre-Register” for your visit. Complete the pre-registration material to streamline the appointment process.

Note: Please call the office where your appointment is scheduled if you do not receive a pre-registration email.

4. Appointment Day

Please arrive 20 minutes before your scheduled appointment time with a photo ID and your insurance cards.
