MCR understands that choosing a new provider can be stressful. Just know, we will be with you every step of the way.
Here’s what you need to do:
2. Schedule Your Appointment
Now that you have selected your provider, call their respective office to schedule your new patient appointment or you can fill out the Request an Appointment form. In the rare case a provider’s schedule doesn’t work for you, our patient services team will assist you in finding the right provider and confirming an appointment time for you.
3. Complete Your Pre-Registration
Two days prior to the scheduled appointment, you will receive an email to “Pre-Register” for your visit. Complete the pre-registration material to streamline the appointment process.
Note: Please call the office where your appointment is scheduled if you do not receive a preregistration email.
4. Appointment Day
Please arrive 20 minutes before your scheduled appointment time with a photo ID and your insurance cards.
We accept private insurance as well as Medicaid and Medicare. If you currently have no insurance, sliding scale fees are available for those who qualify and we will help you apply for coverage.
